What to Expect During TMS Therapy

At NeuroSpa Clinic, every aspect of your TMS experience is designed with care. From the moment you arrive, you’ll receive a brain based therapy that is not only comfortable and non-invasive, but also scientifically personalized using real-time brain data. Our goal is to help you or your loved one feel supported, informed, and empowered at every step.

1. Personalized Evaluation

Your journey begins with a comprehensive intake designed to understand your brain and your overall mental health. This includes:

• A clinical interview to evaluate your current psychological status

• A qEEG brain map to assess your brain’s electrical activity

• A review of symptoms, history, and goals to determine if TMS is the right fit for you or your loved one.

If needed, you may be referred to our consulting neurologist for additional evaluation or diagnostic clarification before beginning treatment.

We believe in using every tool available to ensure the therapy is safe, effective, and tailored specifically to your needs.

2. TMS Mapping and Initial Session

Once you’re cleared to begin treatment, your first appointment will include motor threshold mapping — a key step where we calibrate the TMS device to your brain’s individual sensitivity. This ensures that stimulation is both safe and effective.

Using our EEG-guided protocols, we identify the exact location and strength of magnetic pulses needed to target the brain areas most associated with your symptoms.

You’ll then receive your first TMS session, where you’ll relax comfortably in a reclined chair while a magnetic coil delivers rhythmic pulses to specific brain regions. The procedure is:

Non-invasive and medication-free

Pain-free (you may feel a light tapping sensation)

Quick — lasting about 15 to 40 minutes

After your initial session, we’ll review your response and begin planning your full treatment course.

3. Subsequent Sessions

Following your initial mapping and first treatment, you’ll begin a series of ongoing TMS sessions, typically scheduled 3 to 5 times per week, depending on your condition, goals, and individual needs.

A full course of treatment generally includes 20 to 40 sessions(The U.S. FDA recommends 36 sessions for the treatment of major depression in adults.)

Each session builds on the last — reinforcing healthier brain activity through repetitive stimulation of specific neural networks. Many patients begin to notice improvements in mood, sleep, focus, and anxiety within the first 2 to 3 weeks. Some may start seeing results as soon as the first week. 

Based on your initial evaluation and personalized goals, your treatment may also incorporate concurrent therapies designed to enhance outcomes, such as:

Vagus Nerve Stimulation (VNS)

Meditation or mindfulness exercises

Cognitive training tasks

• Other evidence-based tools that support neuroplasticity

We’ll also be tracking your symptoms weekly using short, secure online forms sent to your phone or email. This allows us to monitor your response in real time and adapt your treatment plan if needed. Likewise, we may conduct additional EEG brain mapping to understand how your brain is responding to TMS treatments.

During each session:

• You’ll sit comfortably in a quiet, focused environment

• Sessions typically last 20–40 minutes. For most people, you can be in and out within 30 minutes. 

• There’s no sedation or recovery time — you can return to daily activities immediately. You can drive to and from your sessions without worry. 

4. Maintenance & Continuing Care

While many patients experience significant improvement after completing their initial course of TMS, mental health is an ongoing journey — and we’re here to support it every step of the way.

Depending on your condition and how your brain responds to treatment, we may recommend a maintenance TMS plan to help sustain your progress over time. Maintenance therapy is tailored to your needs and may include:

Booster sessions (e.g., 1–4 per month) to reinforce neural patterns

Seasonal or situational follow-ups during times of stress or relapse risk

• Continued use of supportive therapies such as neurofeedback, mindfulness training, VNS, or cognitive training

We’ll work with you to develop a personalized maintenance strategy that keeps your brain in balance and helps you stay on track — mentally, emotionally, and functionally.
